Anesthesia and Surgery Consent Form

Dear Client,

Your pet is scheduled for an upcoming surgery. To help make the surgery day as easy and stress-free as possible, we have put together some simple forms for you to review and sign in advance. In addition to this letter, the following forms are available on our website:

• Pre-Anesthetic Blood Testing Information Form
• Surgical Information Sheet

Please review all forms carefully. If you have any questions, feel free to call or email the hospital at (415) 859-5676 or staff@thecitypethospital.com. On your pet’s surgery day, you will be asked to review and sign an Authorization/Estimate Form. We will also confirm a phone number where you can be reached during your pet’s procedure.

The night before your pet’s surgery, please refrain from giving any food or treats after 10:00 p.m. Water is fine and may be left out. If your pet is currently taking any medications, vitamins, or injections, please withhold the morning doses unless otherwise directed by the doctor.

At the time of your pet’s scheduled surgery, the doctor will address any remaining questions and collect your completed Authorization Form.

If you have elected to have any recommended blood tests performed, these will be collected before surgery unless they have already been completed. If any questions arise, the doctor may contact you using the number provided on the Authorization Form.

Once surgery is finished, the doctor will process your payment, review all discharge instructions with you, and provide a written copy for reference. Please don’t hesitate to ask for clarification if anything is unclear.
